Insurance
We are currently considered an out of network provider, meaning that you pay for your visit at the time of service. We’ll provide you with a “superbill” that you can submit to your insurance for reimbursement. We do not guarantee that your insurance company will reimburse you for your visit.
Payment
We currently take all major credit and debit cards, as well as HSA & FSA.

Some routine tests may be covered by your PPO plan. All visits and testing are covered under flexible spending (FSA) and health savings accounts (HSA).

To ensure availability to all our patients, we have a 24 Hour cancelation policy. You’ll be asked for your credit card when you schedule your appointment to hold your slot.
To cancel an appointment, please provide at least 24 hours' notice by contacting us to reschedule. Cancellations made within 24 hours of the scheduled appointment time will incur a $50 fee.
